Medical opinions play an important role in VA disability claims when a Veteran needs to establish a clear connection between a current medical condition and military service. From a medical standpoint, this connection—often referred to as a “nexus”—must be supported by evidence, clinical reasoning, and a thorough review of records.

An independent medical nexus evaluation focuses on whether the available medical evidence supports a service connection, secondary connection, or aggravation relationship based on accepted medical standards.

Why Medical Opinions Matter

From a medical perspective, a nexus opinion helps address questions such as:

  • Whether a current condition is consistent with documented in-service events, exposures, or injuries

  • Whether medical records support a continuity or progression of symptoms

  • Whether one condition may have caused or aggravated another

The strength of a medical opinion depends on evidence and reasoning, not outcomes.


Independent Medical Nexus Evaluations

An independent medical nexus evaluation is a records-only medical review conducted by a licensed medical provider who is not involved in a Veteran’s ongoing care. The purpose of the evaluation is to determine whether a medical nexus opinion can be supported based on the evidence provided.

These evaluations are commonly considered when:

  • A claim was denied due to “no service connection”

  • A prior C&P opinion did not fully address the medical history

  • Secondary or aggravation conditions are involved

  • There are gaps in records that require medical explanation


What a Nexus Evaluation Involves

A comprehensive nexus evaluation typically includes:

  • Review of service treatment records and relevant post-service medical records

  • Analysis of diagnoses, timelines, and clinical findings

  • Application of medical reasoning consistent with VA probability standards

  • A written medical nexus opinion when medically appropriate

Not every evaluation results in a written opinion. Medical conclusions are based solely on evidence.
