How We Define Highly Complex Specialist Cases
The Highly Complex Specialist Pathway is reserved for cases requiring our most comprehensive level of independent medical analysis together with specialty-specific physician review.
Cases may include:
- Numerous claimed conditions
- Multiple interconnected medical conditions or medical relationships
- Complex secondary service connection or aggravation analysis
- Extensive longitudinal medical records
- Multiple prior VA decisions or medical opinions requiring review and reconciliation
- Conflicting medical evidence requiring substantial analysis
- Multiple theories of service connection or complex medical causation questions
- Medical questions requiring review by a U.S.-licensed physician with expertise in the applicable medical specialty
The number of claimed conditions alone does not determine the Specialist Pathway level. Final determination is based on the overall scope and complexity of the medical analysis required, the medical questions presented, and the specialty-specific review appropriate for the case.
Specialist assignment is based on provider availability, applicable specialty, case scope, and appropriateness of the requested review.
