What Should Veterans Look for in a Comprehensive Independent Medical Evaluation?

Understanding What Distinguishes a Comprehensive Independent Medical Evaluation

Choosing an independent medical evaluation provider is an important decision. Veterans often compare providers based on price, turnaround time, or the length of the final report. While those factors may influence a decision, they do not necessarily reflect the quality or scope of the medical evaluation.

A comprehensive independent medical evaluation involves much more than preparing a written report. It is a professional medical assessment based on review of the available medical evidence, accepted medical principles, clinical reasoning, and the reviewing provider's independent medical judgment.

Every veteran presents a different medical history, different diagnoses, and different medical questions. Because of this, comprehensive independent medical evaluations are developed according to the specific medical facts of each individual case.

The purpose of this resource is to help veterans understand what distinguishes a comprehensive medical evaluation so they can make informed decisions when comparing providers.

Information and Professional Medical Judgment

Educational resources can help veterans better understand medical evidence, organize records, and become more informed about the VA disability claims process.

Professional medical evaluation serves a different purpose.

An independent medical evaluation requires application of medical knowledge to the individual veteran's medical history, diagnoses, documented clinical findings, and supporting evidence. These determinations rely on the reviewing provider's education, training, clinical experience, and professional judgment.

Because every veteran presents different medical facts and different medical questions, comprehensive independent medical evaluations cannot be reduced to a standard process, checklist, template, or AI prompt.

Understanding the Medical Questions

Every claim presents unique medical questions.

Depending on the circumstances, those questions may involve:

  • Direct service connection

  • Secondary service connection

  • Aggravation

  • Prior VA denials

  • Toxic exposure

  • Dependency and Indemnity Compensation (DIC)

  • Other medically complex issues

A comprehensive medical evaluation considers the specific medical questions together with the available medical evidence.

Review of the Medical Evidence

A comprehensive evaluation begins with careful review of the medical evidence submitted for consideration.

Depending on the circumstances, records may include:

  • Service Treatment Records (STRs)

  • Military personnel records

  • VA medical records

  • Private medical records

  • Compensation and Pension (C&P) examination reports

  • VA Rating Decisions

  • Diagnostic imaging

  • Laboratory findings

  • Specialist consultation records

  • Operative reports

  • Physical therapy documentation

  • Relevant lay statements

  • Other supporting documentation

The records considered depend on the evidence available for each individual evaluation.

Professional Medical Assessment

Reviewing records is only one part of a comprehensive medical evaluation.

The reviewing provider evaluates the available medical evidence within the context of the veteran's medical history, the medical questions presented, accepted medical principles, and professional clinical judgment.

Every evaluation reflects the specific medical facts of the individual case.

Evidence Based Medical Assessment

When appropriate, the reviewing provider may consider relevant peer reviewed medical literature, accepted clinical guidelines, consensus statements, or other recognized medical references.

These resources may help inform the medical assessment when applicable to the medical questions presented. They do not replace evaluation of the veteran's medical evidence.

Independent Medical Judgment

Independent medical judgment is one of the defining characteristics of a comprehensive medical evaluation.

The reviewing provider develops an independent medical assessment after considering the available medical evidence, accepted medical principles, professional experience, and the individual facts of the case.

The conclusions expressed in the evaluation reflect the reviewing provider's own medical judgment.

The Written Medical Evaluation

The written medical evaluation communicates the reviewing provider's medical analysis, clinical reasoning, and conclusions in a clear, organized, and evidence based manner.

The report documents the completed medical evaluation. It reflects the professional assessment performed during the evaluation.

Every Evaluation Is Different

No two veterans have identical medical histories.

Military service, diagnoses, treatment history, prior VA decisions, diagnostic studies, and supporting medical evidence differ from one individual to another.

For that reason, the medical issues evaluated, the evidence considered, and the professional analysis differ from case to case.

Questions Veterans May Wish to Consider

When comparing independent medical evaluation providers, veterans may find it helpful to ask:

  • Is each evaluation based on the individual facts of the case?

  • Is the available medical evidence reviewed comprehensively?

  • Are reviewing providers appropriately qualified for the medical questions presented?

  • Does the organization explain its evaluation process clearly?

  • Does the evaluation demonstrate evidence based medical reasoning?

  • Does the provider exercise independent medical judgment?

  • Does the organization provide educational resources that help veterans better understand independent medical evidence?

  • Does the organization communicate clearly about what veterans can expect throughout the evaluation process?

These questions may help veterans compare providers based on the overall quality of the medical evaluation.

Why the Evaluation Matters

The written report is the documented result of a comprehensive professional medical evaluation.

Its value comes from careful review of the available medical evidence, professional medical assessment, clinical reasoning, and the reviewing provider's independent medical judgment.

Veterans comparing providers should consider the quality of the medical evaluation as a whole.

Our Commitment

Educational Notice

This resource is intended to help veterans better understand comprehensive independent medical evaluations and the factors they may wish to consider when comparing providers.

Independent medical evaluations require individualized review of the available medical evidence and the independent medical judgment of an appropriately qualified healthcare provider. This resource is educational in nature and should not be interpreted as a guide for preparing medical opinions.

Key Takeaway

A comprehensive independent medical evaluation is defined by the quality of the medical assessment, thoughtful review of the available medical evidence, sound clinical reasoning, and the reviewing provider's independent medical judgment. The written report documents those professional conclusions. It does not replace the evaluation itself.

Terms and Scope of Services Important Notice Current service descriptions, pricing, and terms published on this website supersede prior versions. Independent Medical Nexus provides independent, records-based medical review services for administrative, legal, and disability benefit purposes, including matters involving the U.S. Department of Veterans Affairs (VA). Services may include a Medical Evidence Review or an Independent Nexus Evaluation, depending on the service purchased and accepted for review. Reviews and evaluations are conducted by qualified, doctorate-prepared, U.S.-licensed medical clinicians. When a Specialist Physician Nexus Evaluation is selected and accepted, the evaluation is performed by a U.S.-licensed physician matched according to the medical question, relevant specialty expertise, case scope, licensure requirements, provider availability, and individual case acceptance. Independent Medical Nexus is not a treating medical practice. Services do not include medical examinations, diagnosis, treatment, prescribing, follow-up care, telehealth services, or ongoing medical management. The reviewing medical provider does not establish a physician-patient or other treating provider relationship with the client, does not assume responsibility for the client's medical care, and does not replace the client's treating healthcare provider(s). All medical findings and opinions reflect the independent professional judgment of the reviewing clinician based on the records and information available for review. No finding, recommendation, or opinion is predetermined or guaranteed. Medical Evidence Review The Medical Evidence Review is a stand-alone, records-based professional service designed to evaluate the available medical, service, exposure, and supporting evidence. The purpose of the review is to identify relevant medical issues or evidentiary gaps and determine whether the available evidence appears medically supportable for advancement to a full Independent Nexus Evaluation. The Medical Evidence Review includes written findings and recommendations. It does not include a Nexus Letter or Independent Medical Opinion. A Medical Evidence Review may determine that: • the available evidence supports proceeding to a full Independent Nexus Evaluation; • the available evidence does not currently support proceeding; or • additional evidence may be needed before a full evaluation is appropriate. The Medical Evidence Review is a separate professional service and its fee is not credited toward a later Independent Nexus Evaluation. Independent Nexus Evaluations Independent Nexus Evaluations involve independent review of the submitted records and evidence to determine whether a written medical opinion can be medically supported. When medically supportable, the completed service may include a written Independent Medical Opinion or Nexus Letter. No clinician is required to issue a favorable opinion or any opinion that is not supported by the available medical evidence. Findings or opinions may be favorable, neutral, unfavorable, inconclusive, or declined when the available evidence does not support a medically reasonable conclusion. General Scope of Services Services are limited to the medical records, service records, exposure information, and other supporting documentation voluntarily submitted for review. No in-person, virtual, or telehealth examination is provided unless specifically stated otherwise in writing. No diagnosis, treatment, prescription, or ongoing medical care is rendered. No provider-patient relationship is established. Independent Medical Nexus does not access VA systems, retrieve records on a client's behalf, or submit documents to the Department of Veterans Affairs. Independent Medical Nexus does not provide legal advice, claim representation, advocacy, or legal services. Records Policy Clients are responsible for submitting the records and information they want considered as part of the purchased service. The review is limited to the condition(s), medical question(s), and scope accepted for review. Additional conditions, substantially expanded medical questions, revisions, addendums, or significant new records submitted outside the accepted scope may require additional review and may involve an additional fee. Independent Medical Nexus will notify the client before performing work that requires an additional service or fee. Electronic Delivery and No Physical Shipping Independent Medical Nexus provides professional records-based medical review services and electronically delivered work product. No physical product is shipped. Written Medical Review Findings, Independent Medical Opinions, Nexus Letters, recommendations, receipts, notices, and other service-related documents may be delivered electronically through the email address provided by the client, a secure electronic portal, secure file link, or another electronic delivery method designated by Independent Medical Nexus. Clients are responsible for providing and maintaining accurate contact information, monitoring the email address used during intake, including spam or junk folders, and retaining copies of electronically delivered documents. Electronic delivery to the email address, secure portal, or other electronic delivery method designated for the client constitutes delivery of the completed work product. If a client reports a reasonable technical problem accessing an electronically delivered document, Independent Medical Nexus may provide another electronic copy using an available delivery method. No Physical Returns Because Independent Medical Nexus provides professional services and electronically delivered documents, no physical return process applies. Refund and cancellation eligibility is governed exclusively by the Payment and Refund Policy, the service-specific terms applicable to the purchased service, and the Independent Medical Nexus Service Agreement and Acknowledgment. Client Responsibilities Clients are responsible for submitting any completed Medical Review Findings, Independent Medical Opinion, Nexus Letter, or other documentation to the Department of Veterans Affairs or to any VSO, accredited representative, or attorney assisting with the claim. Independent Medical Nexus does not guarantee that the Department of Veterans Affairs will accept or assign any particular evidentiary weight to documentation provided. Payment and Refund Policy Full payment is required before review begins. The Medical Evidence Review is non-refundable because the service includes professional review time, medical analysis, and preparation of written findings and recommendations. The Medical Evidence Review fee is for the professional work performed and is not dependent upon whether the review supports a favorable nexus conclusion or recommends proceeding to a full Independent Nexus Evaluation. If the Medical Evidence Review determines that the available evidence does not support proceeding to a full Independent Nexus Evaluation, the written findings and recommendation constitute completion of the purchased service. For Independent Nexus Evaluations, cancellation requests received before records are submitted may be eligible for a refund, less a $350 administrative fee to account for intake, processing, case setup, and administrative services associated with the purchase. Once records are submitted, the evaluation enters the review workflow and the evaluation fee becomes non-refundable. Because services consist of professional review and electronically delivered work product, non-use of, disagreement with, failure to submit, or a decision not to rely upon completed findings or opinions does not create a right to a refund. Unauthorized chargebacks may be contested and do not alter financial responsibility for services purchased or professional work performed. No Outcome Guarantees Independent Medical Nexus does not guarantee: • issuance of a favorable medical opinion; • issuance of a Nexus Letter or Independent Medical Opinion; • VA acceptance of any medical opinion; • service connection; • a particular disability rating; or • approval of any VA claim. All VA claim determinations and disability ratings remain solely within the authority of the Department of Veterans Affairs. No medical opinion will be issued that the reviewing clinician determines is not medically supported by the records and evidence reviewed. Service Agreement and Acknowledgment Full service-specific terms are presented in the Independent Medical Nexus Service Agreement and Acknowledgment provided with the intake process. By purchasing a service and completing the applicable intake and acknowledgment process, the client agrees to the terms applicable to the service purchased.
