Highly Complex Evaluation
Our Most Comprehensive Independent Medical Analysis
Records-Only Independent Medical Opinion for Highly Complex Cases
Full Description
The Highly Complex Evaluation is our most comprehensive level of records-based Independent Medical Opinion review.
This evaluation is reserved for medically involved cases requiring extensive independent medical analysis across numerous medical issues, substantial records, complex medical relationships, or multiple theories of service connection.
A case may require this level because of the breadth and depth of the medical analysis involved, regardless of the number of claimed conditions.
Cases commonly appropriate for this evaluation may involve:
- Numerous interconnected claimed conditions
- Extensive longitudinal medical records spanning many years
- Multiple prior VA denials or medical opinions requiring review and reconciliation
- Multiple theories of service connection
- Complex secondary service connection or aggravation relationships
- Multiple medical systems or overlapping medical issues
- Complex medical causation questions requiring extensive analysis
A U.S.-licensed, doctorate-level medical provider conducts an in-depth review of the submitted service and medical records and independently determines whether the available medical evidence supports a written Independent Medical Opinion.

Important Information
This is a records-only independent medical review service. No appointment, physical examination, or telehealth visit is included or required.
You are purchasing the professional review and analysis of your submitted medical and service records and preparation of a written Independent Medical Opinion based on the findings of that review.
Medical opinions are independent and evidence-based. Depending on the evidence reviewed, the opinion may be favorable, neutral, unfavorable, or declined when the available medical evidence does not support the requested medical conclusion.
This service does not include medical diagnosis or treatment, legal advice, claim preparation, advocacy, or VA representation.
Payment becomes non-refundable once submitted records have been accessed and medical review has begun.
All medical conclusions are independently determined by the reviewing provider based on the evidence available for review. All VA disability claim and rating decisions are made solely by the Department of Veterans Affairs. No specific service-connection determination, disability rating, or claim outcome is guaranteed.
How We Determine Highly Complex Cases
The Highly Complex Evaluation is not determined solely by the number of claimed conditions.
The appropriate evaluation level is based on the overall scope, breadth, and complexity of the medical analysis required.
Factors may include the number and relationship of medical conditions, volume and duration of medical records, prior medical opinions, conflicting evidence, secondary or aggravation relationships, multiple theories of service connection, and the complexity of the medical causation questions presented.
If you're unsure whether your case requires a Highly Complex Evaluation, begin with our complimentary Case Readiness Assessment. We'll recommend the evaluation level that best matches the anticipated scope of medical analysis before you proceed.
